Graduate forms

Graduate Studies Office (GSO) forms are regularly updated. You should make sure that you are using the latest version by obtaining forms from this page ahead of each new application.

Please find below application forms that you will need to complete for each milestone stage of your course. In addition to the transfer of status, confirmation of status, examination and submission-related applications, you can also find below applications to change your programme of study, update your personal details, and change your thesis title, as well as other application forms which may be relevant to you during your studies at Oxford. It is your responsibility to submit applications in good time. If you are unclear on the submission/approval process for a particular GSO form or if you have questions about the sharing of your personal data, please contact your Graduate Studies Assistant or departmental graduate administrator.

If you find yourself in exceptional circumstances and require appropriate adjustments to your study timescales and arrangements, following discussions with your supervisor and College Advisor, you will need to complete a form on the Graduate forms for exceptional circumstances webpage.

* Please note: a number of these previously paper-based forms have now been replaced by on-line applications available via Student Self Service under the My Student Record tab. The links have been updated accordingly.

Transfer of status

Refer to the Key milestones for DPhil students webpage or the Key milestones for MLitt and MSc by Research webpage for specific guidance on the process for and timing of the transfer from PRS to DPhil/MLitt/MSc (Res) status. The Examination Regulations and your course handbook will also provide further information on the process.

Confirmation of status

For students admitted before October 2018

Application for Appointment of Examiners MPhil Law and MSt in (Socio-) Legal Research (to be submitted to the Law Faculty). Students of MPhil Law and MSt (Socio-) Legal Research should submit the GSO.27 form instead of the GSO.3

Forms for the submission of hardbound library thesis copies

GSO.26Information for thesis cataloguing (to be submitted to the Bodleian along with the physical thesis). For any student submitting a hardbound library copy for the Degrees of DPhil, MLitt, MSc by Research, BPhil, MPhil, or MSc by coursework.

Dispensation from statutory residency
Graduate offer-holders should download the form using the link provided and, once complete, email it to the relevant Graduate Studies Assistant.

This form is for use by students who require adjustments to the assessment arrangements for their Transfer of Status, Confirmation of Status, and the final oral examination.
